I/My participating child hereby agree to abide by and support the current WAXERS/OMHA rules of play and personal conduct. I/My participating child am/is aware that checking from behind is extremely dangerous and is absolutely prohibited by the WAXERS/OMHA rules of hockey. In addition, an offender may be suspended from playing. I hereby understand and appreciate that participation as a hockey player carries risk to me/my participating child of serious injury, including permanent disability, paralysis or death. I/My participating child voluntarily and knowingly acknowledge, accept and assume these risks. I hereby authorize an official of the WAXERS to have any medical attention performed, should it be necessary in my absence. I the participant/parent reserve the right to take action against any wrongdoer but, subject to that, I release the WAXERS/OMHA and including all members, coaches, managers and association executives from any responsibility. I hereby understand and appreciate limitations on cancellation and refunds. Participants may cancel without penalty 21 days before a clinic starts. Cancellation after this date and up to the clinic start day will incur a non-refundable $50 administrative fee. Once the clinics start, there are no cancellations. Pro-Rata refunds are only provided if a player is unable to participate due injury sustained as a result of participating in this clinic. To apply for a refund, please write the association, include a brief explanation and a doctor's note indicating player is unfit for participation due to injuries sustained in this clinic. I, the undersigned, certify the information provided is true and in consideration of the granting of this registration to me with the privileges incident thereto and by signing this registration I have become subject to the rules, regulations and decisions of the WAXERS/OMHA/OHF/CHA and it’s executive which may be restrictive in some areas such as movement from team to team, conduct, etc. and I agree to abide by such rules, regulations and decisions of the WAXERS/OMHA/OHF/CHA and it’s executive. I am aware that these rules and regulations are available to me through my local executive.
