AGREEMENT TO PARTICIPATE IN THE PROGRAMS OF THE
MARKHAM MINOR HOCKEY ASSOCIATION (MMHA)

A MEMBER OF THE ONTARIO MINOR HOCKEY ASSOCIATION (OMHA)

All registrants are required to complete a separate Canadian Hockey Association (CHA) Players Registration Certificate and attach a proof of age, if required. Certificates to be available at the beginning of each season. The CHA certificate MUST be completed no later than November 30th of the current season. Failure to comply could result in the suspension of play.

I/My participating child hereby agree to abide by and support the current MMHA/OMHA rules of play and personal conduct. I/My participating child am/is aware that checking from behind is extremely dangerous and is absolutely prohibited by the MMHA/OMHA rules of hockey. In addition, an offender may be suspended from playing.

I hereby understand and appreciate that participation as a hockey player carries risk to me/my participating child of serious injury, including permanent disability, paralysis or death. I/My participating child voluntarily and knowingly acknowledge, accept and assume these risks. I hereby authorize an official of the MMHA to have any medical attention performed, should it be necessary in my absence.

I the participant/parent reserve the right to take action against any wrongdoer but, subject to that, I release the MMHA/OMHA and including all members, coaches, managers and association executives from any responsibility.

I, the undersigned, certify the information provided is true and in consideration of the granting of this registration to me with the privileges incident thereto and by signing this registration I have become subject to the rules, regulations and decisions of the MMHA/OMHA/OHF/CHA and it’s executive which may be restrictive in some areas such as movement from team to team, conduct, etc. and I agree to abide by such rules, regulations and decisions of the MMHA/OMHA/OHF/CHA and it’s executive. I am aware that these rules and regulations are available to me through my local executive.
